In order to provide effective patient centered care, frontline healthcare staff must use evidenced-based approaches that promote behavior changes. Staff members are increasingly being asked to practice health coaching, an evidenced-based approach, when providing coordinated care.
This course is part two of a two-part series that aims to provide an overview of the main concepts of health coaching, while also exploring the ways frontline healthcare staff can use Health Coaching to deliver care coordination services.
According to the National Institute of Diabetes and Digestive and Kidney Diseases, 50% of patients do not take their medicines as prescribed. Trusting relationships, with both patients and other care team members, are key to better healthcare outcomes. In continuation of the health coaching two-part series, participants will be provided a deeper dive into health coaching techniques in relation to behavior change, action plans, and medication reconciliation. In this course, healthcare staff will learn how to gain the trust of patients and peers alike, as well as, learn about the integration of care coordination responsibilities into patient care. Participants will explore the best way to performing medication reconciliation. A discussion and demonstration of how to ensure patient understanding, a process called “closing the loop”, will be reviewed.

Learning Objectives:
- Increase understanding of the techniques of health coaching to aid in the completion of care plan goals
- Demonstrate basic understanding of health coaching techniques such as making behavior-change action plans with the patient, confirming the patient understands what the provider has asked them to do and medication reconciliation
